Typical assignment for days and pm shift for RN or LPN is 16 to 18 residents with 2 CNA’s.

*Sets up, administers, monitors and charts oral, intramuscular and other medications and treatments.*Observes residents for changes in physical and/or emotional condition and informs supervising Registered Nurse. Charts observations. Notifies families when condition changes.*Performs/takes and charts residents’ blood pressure, temperature, blood sugar, pulse and respiration.*Converses with patients to assure they are comfortable, explains therapies, treatments, medications and changes in same. Answers resident’s request for assistance.*Answers questions from family members.*Reassures and provides emotional support to residents.*Takes and records physician’s telephone medical orders.*Charts nutritional intake, bowel movements, fluid intake/output, and other resident activities such as leaving the facility.*Reports needs or changes to other facility departments such as maintenance, dietary, housekeeping, laundry, physical therapy, occupation therapy, pharmacy and beautician.*Assists maintaining supplies and medication inventories. *Maintains knowledge of nursing principles, methods and standards; and health care, sanitation, hygiene and medical treatment.*Attends care conferences, in-service training, and staff and facility meetings. *Directs visitors.*Monitors alarm system for wandering residents. Redirects wandering residents.RNThe purpose of positions in this classification is to evaluate resident physical and mental status, obtain and implement physician’s orders; supervise subordinate nursing staff and notify appropriate persons regarding resident change in condition.
1. Evaluates residents for response to treatments and medications. Monitors resident vital signs. Reports change in status to physician. Obtains and implements physician’s orders.2. Supervises work performance of subordinate nursing staff for proper technique and use of equipment, materials and supplies. Prepares written evaluations. Conducts new employee orientation. 3. Maintains complete and accurate resident charts for treatments, medications, etc.4. Administers medications orally, intramuscularly, subcutaneously, intradermally, rectally and intravenously. Administers fluids intravenously.5. Prepares and implements resident care plans. 6. Converses with residents to assure they are comfortable, explains therapies, treatments, medications and changes in same. Answers resident’s request for assistance.7. Answers questions and complaints from family members.8. Reassures and provides emotional support to residents.9. Accompanies physicians during rounds.10. Attends and participates staff meetings, in-service and continuing education.11. Orders medications, supplies and materials.12. Responsible to know and practice the Safety policies of the County.
