ESSENTIAL DUTIES AND RESPONSIBILITIES: These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned. 1. Provides case management and crisis intervention services to children, youth, guardians, and adoptive parents to achieve the best possible outcomes and minimize disruptions. 2. Possesses the ability to provide services in-home, out-of-home, and through virtual appointments to accommodate family’s needs. 3. Builds upon knowledge of evidence-based interventions to support permanency-focused interventions. 4. Provides psychoeducation to empower the family system with the needed tools to strengthen and nourish informal and formal supports. 5. Possesses culture humility and the ability to build trusting relationships with vulnerable and diverse populations. 6. Maintains trusting relationships through effective and meaningful contact with children, youth, and their care providers. 7. Respects the dignity and worth of children, youth, guardians, and adoptive parents. 8. Identifies and understand the strengths, abilities, and competencies of the child, youth, guardians, and adoptive parents. 9. Assesses individual needs of the family, makes referrals to community services, and advocates for their best interest. 10. Assesses the familial situations and provide information to the courts. 11. Assists the family in any court matters relating to guardianships or adoptions. Court reports and testimony is required. 12. Demonstrates effective written and verbal communication skills. 13. Provides assistance to the Kinship Coordinator to include application process and documentation in eWiSACWIS. 14. Develops and facilitates of the Relative Caregiver Support Group and Permanency Support Group, or other groups to support children, youth, guardians, and adoptive parents. 15. Knowledgeable in Department of Children and Families Administrative Code 56 (Foster Care Licensing), Code 58 (Kinship), and SAFE Home Study assessments. 16. Demonstrates excellent organizational, time management and writing skills. 17. Supports and advocates for a consistently developing agency culture of hope and resilience. 18. Adapts to change in an ever-changing and fast-paced environment. 19. Functions as a team member with involved agency staff. Possesses ability to work across several work units and develop positive relationships. 20. Participates in a rotating paid on-call system to respond to emergent situations outside of traditional business hours, including weekends and holidays. 21. Responsible to practice the Safety policies of the County. Perform all job tasks in a safe and prescribed manner. 22. Must be familiar with and exemplify the Wood County Core Values and serve with courtesy and respect in accordance with the Wood County Code of Conduct. B. Other Duties 1. Attend agency staff meetings, appropriate training and education, and supervision. 2. Demonstrate the ability to critically think by constructively utilizing training and the education provided as well as supervisory consultation. 3. Represents the Department in the community through speaking engagements and by participation on boards and committees. 4. Other duties as assigned.
